Fable III did it, but in such a shallow way that literally the only thing that changed was the gender of your character's Hero parent.
I understand why you might do it for an actual trilogy like Mass Effect, but I don't think there's much need to do it for Dragon Age, when they've decided to have each game taking place years apart and in different cities, GTA style. Plus the writers seem to have their own canon for it anyway.
Skyrim didn't have save-game import, and that managed all right, after all.

What if the reapers are just the beggining of it all. Maybe we will realize the greater threat beyond the reapers. What if something else is out there? What if Mass Effect 3 is just the end of the first part of the trilogy?

I've never heard that song before. That was awesome. xD Have you never heard that? It's from the musical special episode of 'Buffy the Vampire Slayer'. Easily the best episode if you ask me, and it's a perfectly good musical in its own right. Very good, very catchy tunes and a good plot.

I dont know other games which have done this save besides Mass Effect, Dragon Age and the Witcher. Anyone else? I'm a bit late, but I know that Baldur's Gate 2 did this before and last I've heard, it did mostly well. Better than most of the current save transfer shiktick nowdays.

Man, good question. I still have the game but it's been so long since I played it that I don't remember if they actually die or not. It might be negated if a cleric can rez though.

The Wizardry series is well known for this. If I'm not mistaken, they're the ones who pioneered it.

I don't know if it's a good idea to continue Mass Effect franchise (unless they plan to create a DLC for better ME3 endings).
For example, now that the Reapers are done for, what the new heroes will have to fight against? An intergalactic invasion? How will the asari, turians, humans, etc. cooperate with each other now that the mass relays are destroyed? There are too many unknowns for this kind of sequel.

Hence this is way there's a big movement from the fans to change the endings. Not only the current ending is left with too many plotholes and more questions, but also it's a potential killer series. There's no way BioWare can continue the ME verse without a hitch. Besides, there's no "Mass Effect" in the name without the said relays and ME technology.
I'll won't be buying the ME4 game on the first day if they keep the ending in it's current state. It would be like WoW all over again, with the constant awful retcons that Blizzard did in the late.
