Fallout 3 marked the last time I was truly frightened playing a game. I was doing just what Mr. Glow was doing: Just minding my own business, wandering around the Wasteland when I happen to pass by a town called Old Olney. There was a Vault on the other side of it that I wanted to explore, so I passed on through.
The first deathclaw attack was a surprise and I was completely thrown off guard by it. After I recovered, I weave my way into the middle of town, and only then did I realize...

But speaking of Overlord, I really think I should head over to Gamestop and pick up some of them Microsoft Points so's I can download it.
And Renegade, this might help - or make things worse, I dunno:

I'm very easily scared - but actually the first time I saw a centaur I laughed. I love those little guys. The super mutants' faces kinda creeped me, though. Something about their expressions.

Super Mutants are the "Goddamn Bats" in the Fallout 3 universe in eyes. Althought I do admit when that Giant Fat Ass super mutant came out in the middle of nowhere after I've acidently trigger the trap (me being the typical Schmuck Bait) I was screaming while running away.
However Point Lookout was scary to me. I don't know, the lack of hospitality along with the isolated deserted place makes it seem eerie to me. Doesn't help that I've been chased by red neck mutants who seem to taken the invincible crack in their spare time.

One of the creepiest parts of Point Lookout was during the Dark Heart of Blackhall quest, you return to the nun's tent and find her dead, with stuff written on the walls in her blood.
And I don't kow if anyone's mentioned Vault 87 yet, but that place was so messed up. Lots of centaurs and confined spaces with the walls and floors coated in gore. Yuck.

Oh god... don't get me started with vault 87. That vault... my god, not it's scary but at the same time... I feel really bad for the dwellers in vault 87. Reading their logs just... horrible.
ah... vualt Gary. It's quite unevering at first but the constant battle cry that is consist of "Gary, gary GAAARY!" made me felt a little amused at least. And I've played a little counting game too: "Ooooh! One beaded Gary! And here's another Gary that is using a lead pipe, trying to smash my skull and etc."
Another vault I've find it unerving is the Music vault. I think it's vault 92. It's the emptiness, white noise and the sad notes that were left behind by dead musicians kinda hit home to me...

never actually had a problem with fallout 3, always felt that no matter what i came across, it wouldn't be anything the old plasma rifle couldn't turn into a pile of ashes.
It may seem like an obvious choice, but id have to go with silent hill 3 myself. SH2 was perfect psychological horror, the dense plot making it haunt you long after the console was switched off, but for more direct, visceral fear, SH3 was the one, everything from the sound (can still hear the flying things from the mall area, the ones that sounded like two pieces of rusty sheet metal rubbing together when they saw you) to the environments (blood and rust for everybody!) it also went out of its way to create some of the most unsettling moments i have ever experienced playing a videogame.

I can't believe I forgot Doom 3. That was the first and only time I got killed in a game because I knocked the mouse off the desk XD It doesn't help that half the game areas are pitch black. And you have to switch back and forth between the flashlight and your gun. I don't care who you are, that game will get you out of your seat one way or another. Pictured: One of the least terrifying moments in the game. One of my favorite is when you go to Hell, and you see the shadow of a hell knight. Then he turns the other way and leaves. "Phew" you think, as you just fought off half of hell's legion single handedly and you're a little low on ammo. So you turn around and OMFG AHHHH!!!! *fires every bullet* Or one of the thousands of "Hmm whats in this ominous yet strangely empty room, must be a SECRET AREA. Nothing? Oh well, guess I'll just *the blackest of blackness descends upon you. Suddenly, some crazy ass satanic pentagram appears in the center of the room spewing flame and all manner of weird shit as an army of imps materialize around you helpless soon-to-be-corpse* This game is while Tim Shaufer fail in his "metal" game. Doom is heavy metal purified into video game form. Ready to be injected straight into your brain. Every frame is like a memory imprinted from a nightmare. Every sound reminds you of when you were a child, in your room late an night, cringing at every creek you heard. It reminds you that you are mortal, one day you will die. And you will burn forever in hell fire. Sorry for the wall of text, I'm really fond of this game.

I was going to say Doom 3, too. I actually wasn;t so keen on the game. But - it did scare the pants off me.

Its best played at night in a pitch black room with surround sound, or good headphones.
Not so good for trying to get to sleep though. There's evil baby locust things under my bed.

Doom 3 was the biggest disappointment in gaming history in my opinion. It had impressive graphics and a good engine, but happened to be quite the sub-par game. I'd take Doom 1 and 2 over it any day (which I happen to do).
Speaking of which, if anybody wants a good scare, I'd recommend System Shock 2. The game is full of good creepy moments.

System Shock 2 is still a fine, fine game. There are some nice tweaking mods out there too now, which improve textures and some of the AI to make it even better. If you've not played this, you really should.

Alan Wake's not a very scary game, but there were a couple of moments which really creeped me out. Throughout the game, a mysterious person has been leaving advice and directions, painted on the wall in light sensitive paint. You can follow the paint arrows to find chests full of useful supplies like batteries, flares and ammo.
Anyway, halfway through the game, Wake has to go through a ruined house in the mountains, and in the basement, you can see loads of creepy paintings and weird messages like 'where did you go?' 'you talk to me on TV' and at least 50 'Toms' written on the walls. It doesn't help that there is a massive ambush when you go upstairs, with about 10 Taken bursting from the shadows.

I've been trying to get System Shock 2 but it's difficult for me to find it. It's just one of those classics where you really HAVE to dig out from obsure game shops like Dungeon Crawl, etc, etc.
