I had a pretty cool unscripted sequence play out in GTA V a couple of days ago.
Basically, there's a point in the story when Michael and Trevor anger a cartel boss and are exiled from Los Santos. I got bored of desert while cruising around as Michael and decided to go back into town. As soon as I get a fair distance into the city, I get a call telling me I was a fool to come back.
Before I could get any goons sent after me, I got arrested, though. The police take all your ammo when you get busted, and as I was walking out of the station looking for a car, a few cars of cartel guys swooped down on me. Without any way of fighting back, I just hid behind a wall, and tried to stay undetected as a large cartel/cop gun battle started.
When enough of the cartel members had been killed, I made a break for it, grabbing some pistol ammo a dead cop had dropped. I ran around the corner, then down and alleyway. Just when I thought I'd got away, a cartel in an SUV spawned around the corner and started driving right down the alleyway towards me. I activated Michael's Bullet Time and managed to shoot him in the head and then roll out of the car's path.
With the cartel still after me and the police on my tail for joining the shootout, I pulled the dead guy out of the SUV, and drove off for Sandy Shores, tearing down the highway with both groups attempting to pursue me.
Definitely one of my favourite moments of the game so far, especially since it was emergent, and not really mission-related.
The only thing that really spoiled the moment was that the SUV's radio was tuned to Non-Stop Pop FM and I had to make my high-speed escape to Rihanna's Only Girl In The World.

Personally, I think "This Fire Burns" from Killswitch Engage would have been pretty fitting.

For a time when the music really suited what I was doing though, at one point I was going around the San Andreas coast in a speedboat at sunset with Baker Street playing:
Strangely enough, I was playing as Michael that time, too. Most of my favourite moments seem to happen while playing as him, despite the fact he's my least favourite and least used protagonist.

(Spoilers) The ending of GTA V.
Things come to a head and it seems like Franklin is going to have to kill off at least one of the other protagonists. Except you can choose to tell the Big Bad to go fuck himself, join up with the other protags, fend off an entire private army and then kill pretty much every major and minor antagonist left alive.
It was a pretty nice change from the last few endings to Rockstar games that normally had a character's death played up for a lot of drama.

Skyrim once more. Just got done making the Thalmor look like chumps when I head back to Riverwood to talk to Delphine. Once I get there, a dragon attacks out of nowhere. Strangely enough, it wasn't me who landed the killing blow on the dragon. It was Gerdur of all people. Just gotta love those fiesty Nord women.

Skyrim for me as well.
Fireball spell + Bandit coming at me with a spear + Wilhelm scream mod + flying off a ledge behind them = The most awesome shit ever.

Vampire the Masquerade: Bloodlines. The Ocean House Hotel level. Need I say more?

Hell it still creeps me a bit and I've been through it dozens of times. Part of me wants to see another Vampire the Masquerade game with updated graphics and less bugs but I doubt it'll ever happen.

Agreed. Crying shame they can't do scary sequences like they used to. Subtle creepiness and atmosphere always outdoes monster closets and grossout moments.
I still stand by the fact that the Thief games had the scariest moments in non-horror games. Particularly the desecrated Cathedral mission in the Dark Project and the Cradle mission in Deadly Shadows.
